Applying to TPP
To apply for the Transition Partnership Program (TPP):
  1. START THE REFERRAL PROCESS
    You should submit a Consent for Release of Confidential Information Form as soon as possible. This is the most important step in starting your referral process. You will receive this form from your special education teacher, or you can download and print the Consent Form ( PDF 20K ) and send it in now!

  2. CHECK OUT THE REQUIREMENTS
    To be eligible for TPP, students must demonstrate that they have:
    1. Barrier to employment in the form of a disability, such as
      • A learning difference, such as dyslexia or ADD
      • A physical barrier, such as visual or hearing impairments or physical limitations
      • A psychological challenge, such as an addiction
    2. Need for services that can be provided by the Department of Rehabilitation (DOR)
    Services are NOT automatic, and the process of determining whether you are eligible is lengthy. We will work with you throughout this process and help determine the severity of your disability.

  3. MEET WITH OUR STAFF
    You will meet with a member of our staff so that we can get to know you personally and help you determine a plan for employment. We will:
    • Give you appropriate paperwork to complete
    • Help you prepare for interviews
    • Set up interviews
    • Attend interviews with you, if appropriate
    • Work with you until you are employed
    • Be available to you during employment if you have questions or concerns
  4. KEEP IN TOUCH
    Once you are employed, we will continue to check in with you and your employer to see how things are going and to help you retain your job.
