All continuing students entering 7th grade and all new and returning students entering 7th through 12th grades will need proof of receiving a booster shot (also known as "Tdap") for whooping cough (pertussis). If you have any questions, please contact your provider or one of the clinics listed in the PAUSD Tdap immunization information letter.

The Health Report for School Entry form may be used to document your immunizations, Mantoux tuberculosis skin test results, and general health exam for first grade only. You may submit equivalent documentation provided by your physician or clinic in place of the Report of Health Examination form. All documentation must be returned to the PAUSD Central Attendance office before your child may enter school.

All documentation MUST be signed or stamped by your physician or clinic and provide the MONTH, DAY, and YEAR of each immunization, along with the date given, date read, millimeters of induration, and impression of the Mantoux Tuberculosis skin test.

PLEASE NOTE: Immunizations and health examinations do not need to be up-to-date in order to begin registration. However, they must be up-to-date and submitted to Central Attendance before the student can receive an admit slip and begin school.

Immunizations

To enter or transfer into elementary and secondary schools (grades kindergarten through 12), children under age 18 years must have immunizations as outlined in the following:

VACCINE REQUIRED DOSES
Polio 4 doses at any age, but... 3 doses meet the requirement for ages 4-6 years if at least one was given on or after the 4th birthday,* 3 doses meet the requirement for ages 7-17 years if at least one was given on or after the 2nd birthday.*
Diphtheria, Tetanus, and Pertussis
Age 6 years and under
(Pertussis is required)

DTP, DTaP or any combination of DTP or DTaP with DT (diphtheria and tetanus)
5 doses at any age, but... 4 doses meet the requirement for ages 4-6 years if at least one was on or after the 4th birthday.*
Age 7 years and older
(Pertussis is not required)

Td, DT, or DTP, DTaP or any combination of these
4 doses at any age, but... 3 doses meet the requirement for ages 7-17 years if at least one was on or after the 2nd birthday.* If last dose was given before the 2nd birthday, one more (Td) dose is required.
Tdap One dose of Tdap vaccine on or after the 7th birthday is required for all students entering 7th through 12th grades in 2011-2012 and all students entering 7th grade in 2012-2013 and future school years.
Tdap, Adacel, Boostrix DTaP or DTP
   
Measles, Mumps, Rubella (MMR) 2 doses** both on or after 1st birthday.*
   
Hepatitis B 3 doses at any age

1 dose for children under 13 years; 2 doses are needed if immunized on or after 13th birthday.****

NOTE: If a student has ever attended a California School before July 1, 2001, the varicella doses are not required.

*Receipt of the dose up to (and including) 4 days before the birthday will satisfy the school entry immunization requirement.

**Two doses of measles-containing vaccine required. One dose of mumps and rubella-containing vaccine required; mumps vaccine is not required for children 7 years of age and older.

***Two doses of the 2-dose hepatitis B vaccine formulation along with provider documentation that the 2-dose hepatitis B vaccine formulation was used for both doses and both doses were received at age 11-15 years will also fulfill this requirement.


****Physician-documented varicella (chickenpox) disease history or immunity meets the varicella requirement.





Mantoux Tuberculosis Skin Test

Tuberculosis Screening Requirement for Santa Clara County Revised 11/14/11

The requirement applies to all students who register for kindergarten for the first time, and all students transferring to Santa Clara County schools into kindergarten through twelfth grade from a school outside of Santa Clara County.

Exemptions: 

All students who have previously met the TB screening requirements of Santa Clara County and who have not been residing outside the county longer than 12 months.

Students transferring from one school to another within Santa Clara County who have previously met the TB screening requirements. 

Documentation is required if transferring within Santa Clara County.

A Student who falls under the provisions of the McKinney-Vento Homeless Assistance Act may be immediately enrolled in school if they have completed a TB sign and symptom review form.  TB screening is still required for these students and should be completed within 10 days of calendar enrollment.  District may extend time to complete screening for up to 45 calendar days.

Acceptable TB tests:

Mantoux Tuberculin Skin Test (TST), which must be done in the United States.  (A 4-pronged Tine multipuncture test is not acceptable). 

Interferon Gamma Release Assay (IGRA) blood test, e.g. Quantiferon or T-spot, which must be done in the United States, for children who are at least 4 years old at the time the blood test is done.

Kindergarten entrants and transferring students from outside Santa Clara County into grades one through twelve must present written evidence of a TB Screening test.  This test must be performed within twelve months prior to first school entrance or transfer.

For TST, the documentation must include:

1. Date the test was given and date the test was read.

2. The result of the reading in millimeters of induration.

3. Signature of the physician or designee. Providers are encouraged to use the yellow California immunization card.

For the IGRA, the documentation must include:

1. Original or copy of lab report.

2. Providers may attach IGRA results to yellow California immunization card, as there is no current place to record the IGRA on the card.

 


The Mantoux skin test MUST be read between 48 and 72 hours after being administered (no more and no less). We recommend that you do not have the test given on a Thursday because it would have to be read on the weekend.

Positive readings:
If your child has a positive reading of 10 mm or greater, your child will need to have a chest x-ray before being allowed to attend school. The chest x-ray must be read and certified by a physician documenting that the student has a normal chest x-ray and is free of communicable tuberculosis.

Students presenting a physician's written documentation of current or previously completed treatment for latent tuberculin infection (six months minimum) for a positive TB skin test shall be considered as meeting the TB skin testing requirement.





General Health Exam (For 1st grade only)
California state law requires students to have a general health examination 18 months prior to entering first grade. The examination can be given up to six months before entering kindergarten, but not before March 15th of that year. Please complete the Health Report for School Entry form.

We suggest that children have a general exam when visiting the doctor for immunization boosters before kindergarten.
